Why? Ran out of parts because the press broke down
Why? Press didn’t receive scheduled maintenance
Why? Maintenance is understaffed
Why? Maintenance department was over budget due to high overtime costs
Why? The company was not reaching profit goals so the CEO had issued an order to avoid all unnecessary spending.
I also Believe having a knowledgeable with the necessary tribal knowledge can make a great impact on what may come. At our company we have failed product launch's due to poor planning and lack of knowledge of how our production line actually operates.
Scenario planning is a strategic planning process where a small number of futuristic stories or scenarios are developed. it being a group process with open knowledge exchange around a main group of issues that may occur, this is beneficial when trying to make a plan work. Especially when the group has knowledgeable people from different departments collaborating together to accomplish a major goal.

i feel like alot of todays problems and when you it break down for a 5 WHY aspect is because there is such a high turn over rate and people shy away from training the new guys knowing they will leave.
scenario planning is unique with the fact that it puts together stories and scenarios to help prevent down time or other scenarios that could stop or slow down producation. adding a production leader and operator can help spread knowledge to other employees to help keep the line moving.
